o The undersigned hereby certifies that the tax licenses are valid and have not been revoked or suspended. The undersigned understands that should they choose to also sell tobacco products, that Provincial Tobacco Tax licenses are required and must be reported to HOUSE OF HORVATH INC. prior to any sale of any Tobacco product.
o The undersigned is responsible to report immediately any changes in status to their tax licenses to HOUSE OF HORVATH INC. The undersigned may be asked at any time for proof of License and must provide said proof to HOUSE OF HORVATH INC. upon request.
o The undersigned acknowledges that as a Tax Collector they are required to charge all appropriate taxes and report all transactions to the appropriate tax authorities as required by law in accordance with the regulations of those tax authorities.
o The undersigned hereby authorizes HOUSE OF HORVATH INC. to conduct a personal and/or financial investigation to obtain information in relation to this application. The signature below hereby authorizes the release of such information from all relevant sources.

TERMS & CONDITIONS AGREEMENT
o PAYMENT: orders can be made by credit card
o TERMS: Initial orders are C.O.D. for a minimum of 3 months and can be made by credit card. 2.5% net 12 days, gross 30 days – subject to approval. Early pay discount does not apply if paid via credit card.
o The undersigned recognizes and agrees to pay a nominal interest charge of 2.5% per month (30% per annum) on any/all overdue balance on account. The assessment and collection of said interest charge in no way commits HOUSE OF HORVATH INC. to future credit privileges. Refusal to comply with this statute will result in a withdrawal of credit terms and a refusal to sell by HOUSE OF HORVATH INC.
o All orders must meet a minimum of $300.00 excluding all taxes or else shipping/freight charges will incur.
o RETURN POLICY – We accept the return of unopened items in their original packaging up to 90 days after purchase date with receipt or proof of purchase at the customer’s expense. Exchanges can be made for items that arrived mis-shipped or damaged if communicated with receipt or proof of purchase to House of Horvath within three (3) days of receipt of goods. Authorization must be obtained from HOUSE OF HORVATH INC. before returning product. Please call the Order Desk at 1-855-604-WILD for approval and shipping instructions. Credit notes are "on account" and issued after receipt of returned goods, once verified by our receiver to the contents, and applied against future order(s).
o No claims on short shipments will be considered unless made within three (3) working days of receipt of goods.
o Credit notes are required for deduction from Accounts Receivable. Debit memos are not accepted in lieu of Credit Notes.
o Prices are subject to change without notice.
o Recipients of our Price List are under no obligation to accept the suggested retail prices.
o HOUSE OF HORVATH INC. reserves the right to accept or refuse the order in whole or in part.
o Acceptance of an order by HOUSE OF HORVATH INC. does not include acceptance by HOUSE OF HORVATH INC. of any terms, conditions, or other provisions appearing on the purchaser's order form or other documents.
o Credit privileges may be suspended or withdrawn at our discretion.
o HOUSE OF HORVATH INC. may terminate this agreement at any time without notice.
